Grey scale ultrasonography of neonatal abdominal masses applies a pattern recognition approach, which identifies the origin and extent and the character of such masses; this, combined with the clinical presentation, may suggest a tissue specific diagnosis. Surgery, when indicated, and the surgical approach are thus better guided. Illustrative cases of neonatal hydronephrosis complicating hydrometrocolpos and proximal vaginal atresia and a splenic cyst are described, other cases summarized and the relevant literature reviewed. Ultrasound is efficacious in the workup of neonatal abdominal masses and a plea for its availability and application in developing countries is made.
